Registration and Forms

We are pleased to announce that Mount Si High School has partnered with FinalForms, an online forms, and data management service. FinalForms works with most of the high schools in Washington State and allows families to complete all of the requirements online. This tool also saves data from season to season and year to year, meaning that you will never need to enter the same information twice.

This will be a requirement to complete for your student to compete in athletics at Mount Si.

Before beginning registration, please make sure you have the following:

Completed physical form
Basic medical and health information
Insurance company and policy number
Doctor, dentist, and medical specialist contact information
Hospital preference and contact information

Additional Registration Information

General Information

Student-athletes must attend school full-time (7 classes) and meet WIAA eligibility standards to participate in sports at MSHS by doctor approval, medical insurance, grades, attendance, residency, behavior, etc.

The clearance process will check these areas to determine eligibility. Only registered and cleared athletes may participate in the first day of tryouts or practice.
