Field Trip Forms

Field Trip Permission Form Now Available in FinalForms

All MSHS students now have a general field trip permission form in FinalForms. Parents/guardians can approve it anytime—either now or when a trip is scheduled. Advisors will create trip-specific groups with dates and details.

Note: Overnight or out-of-state trips require an additional permission form from the trip organizer.

FinalForms Account Setup & Access

In June, all MSHS students and their families were issued FinalForms accounts using data from Skyward Family Access.

Parents/Guardians:

  • Check your email for a FinalForms confirmation message.
  • Once confirmed, log in to complete required forms, including the field trip permission form.

Students:

  • Log in using your school email.
  • Sign all student forms.

FinalForms & Skyward Integration

To improve accuracy and reduce manual entry, FinalForms now syncs automatically with Skyward:

  • Automatic Account Creation: Students in grades 9–12 without an account will have one created using Skyward data.
  • Account Updates: Existing accounts will be updated with student ID and contact info.
  • Confirmation Emails: New users received a confirmation email on June 10 to activate their account.
  • Ongoing Sync: Accounts will be maintained automatically—no manual setup needed.

Annual Updates Required

Even with automatic syncing, families must log in each school year to:

  • Review and confirm student information
  • Complete required forms for athletics and field trips

Please also verify that medical and emergency contact information in Skyward Family Access is current.
